In this study, the natural history and therapeutic management of 30 patients with carcinoma of unknown primary (CUP syndrome) is presented. Despite detailed laboratory investigations, the primary occult tumor was not detected in any of our patients. In addition, no response to either chemotherapy or hormonotherapy was achieved; overall median survival was only 5 months. A thorough review of the literature is available. The philosophy of investigating and treating these patients, as well as the diagnostic accuracy and pathology pitfalls are also discussed.
